Fill-out the form and follow the directions below if you wish to submit a request with one of our city officials.

Directions:

In order to submit a valid request, you must submit your request at least 72 hours in advance. These meetings are 15 minutes long and are scheduled between the hours of 11AM - 3PM on week days, if these times do not work please contact the City Hall.

We also ask that you have 3 days/times available for meeting times.

We ask you to be conscientious of our official's busy work schedules and remind you that they cannot accept all meeting requests submitted. If you have any questions, please call the City Hall @ (812) 265-8300
